ACLD Service Coordination
Medicaid Service Coordination (MSC)
The focus of ACLD’s Service Coordination Program is to assist individuals in achieving their unique goals and dreams. This may include goals to have a home, productive activities or employment, a circle of friends, enjoyable leisure activities or access to medical and clinical services. This program is available to those who are Medicaid-eligible and who can be approved for services through the Office for People with Developmental Disabilities (OPWDD).
Non-Medicaid Service Coordination
This program is provided through a Family Support Services grant and is available to those who are not Medicaid-eligible but who require assistance in accessing necessary services.
Family Member Training
This is a grant program designed to provide information and referrals, life planning assistance and transitional planning awareness to individuals and their families. This program also gives information and referrals to local schools, hospitals and other agencies. ACLD’s Family Support Services Programs assist the families of individuals with developmental disabilities with the goal of maintaining the family members within the family unit.

The Front Door will:
- Train families on the services available
- Determine eligibility of services
- Assess strengths
- Identify support needs
- Plan authorization and implementation
